Volunteer Waiver | Background Check Approval

To protect our neighbors, all Brookside Community Church volunteers must give consent to a criminal background check. Not every opportunity requires a criminal background check. Volunteer opportunities that do require a criminal background check will collect personal information necessary for completing the check. If you do not give your consent, you will not be able to continue with the volunteer application.

I hereby give my permission for Brookside Community Church to obtain information relating to my residence and to conduct a criminal background check. I understand that this information will be used to determine my eligibility for a volunteer position with the Brookside Community Church. I also understand that, if accepted into the volunteer program, as long as I remain a volunteer, the Brookside Community Church may repeat this criminal background check at any time.
